An electronic deprescribing tool that helps physicians and pharmacians through the process of safe and successful medication deprescription in older adults. This project started as a clinical trial with over 6000 patients. It is now used by multiple facilities accross Canada either directly or through integration with the software they already use such as PointClickCare and MED e-care.
